THE EXAMINATION CONSISTS OF
Physical exam
Vaccinations, Tuberculosis* Blood Test, & Gonorrhea Test are an additional fee but are required.
*If the Tuberculosis Blood Test is positive, x-ray may be needed. X-ray service is an additional fee.
The need for vaccinations is dependent on medical history and age, and will be evaluated at the time of your appointment. Vaccinations potentially needed are:
MMR--measles, mumps, rubella
Tdap/DTap
Hepatitis B
Varicella
• Syphillis Test
Influenza--October to March only
Pneumococcal
Meningococcal
HiB
IPV
